Information About The PCR
All
men who are eligible for inclusion on the PCR will receive a letter of
introduction, signed by their Consultant or the Head of the Department in their
respective institution. For example, if
a man was diagnosed at the Austin Hospital, a letter will be sent from the
Austin Hospital and will be signed by the Head of the Urology Service.
Accompanying
the letter will be a Patient Information and Consent Form , outlining the purpose of the PCR
and providing important information to men on how they can opt out, if they do
not wish their details to be retained by the PCR.
For men who are listed as
requiring an interpreter in the patient's medical record, a copy of the
Explanatory Statement is available in the following languages:
